  • Present and former address(es) and Landlord's name(s), and address(es) for the past two years
  • Employer name(s), address(es), phone number(s), length of employment, position, and salary for the past two years
  • Name and address of surveyor, title company or attorney, and homeowner's insurance agent

List Assets:

  • Bank name, address, branch, account number, balance, and name of account holder
  • List all assets and their market value, including real estate, cars, boats, life insurance, stocks, bonds and other investments
  • Other income information (retirement, rental, etc.)

Debts:

  • List all debts: names, addresses, account numbers, and approximate balances (include credit cards, retail stores, credit unions, finance companies, and installment loans)
  • Child support or alimony (documentation of amounts and duration of obligation)
  • Present and past mortgages(s), name and address of mortgage company, loan number, type of loan (VA, FHA, conventional, loan assumption), balance due, original loan amount, monthly payment, and intent to sell or rent property
  • Source of down payment and closing costs
